The Case Manager is responsible for the delivery of care coordination services for our patients in the ambulatory clinic setting. The Case Manager assesses treatment needs, monitoring, and evaluating treatment plans and progress. Case Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. The Case Manager process encompasses excellent communication, both verbal and written,

As a successful candidate you will:

  • Advocates for and assist with coordinating hospice care by working with other team members as well as with the patients/family members to ensure the patient’s needs are met.
  • Assists patients and families with information regarding social, economic and emotional aspects and makes necessary referral.
  • Utilize the nursing process to provide professional nursing care and consult with the physicians and other team members concerning changes in patients’ condition requiring order changes and/or changes in the plan of care.
  • Evaluate the patient, family member (s) and home situation to determine what health teaching will be required and provide education to patients/family members regarding the end-of-life process, plan of care and offers guidance with healthcare decisions.
  • Maintains current knowledge and awareness of organizational and regulatory standards, policies and procedures.
  • Promotes efficient use of health care resources and assumes leadership role with the care coordination and clinic team. This position will contribute to the expansion of City of Hope Lennar Foundation Cancer Center and work with leadership on building the case management program.

Case Manager RN is a full-time role that will be working on a Day Shift, Monday - Friday.

Your qualifications should include:

  • Bachelor’s degree in nursing
  • Three years current clinical experience in the area of case management in the ambulatory/inpatient setting.
  • Current California registered nurse license
  • Current American Heart Association BLS certification
  • Preferred OCN, CCM certifications

Salary / Pay Rate Information:
Pay Rate: $63.11 - $91.51 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
