College/Department Information
Located conveniently in Augusta's downtown medical community, Georgia War Veterans Nursing Home provides continuous professional nursing care to veterans of Georgia who have served in the armed forces during war times. The home is owned and funded by the Georgia Department of Veterans Service and operated through an interagency agreement with Augusta University. Through this association, Georgia War Veterans Nursing Home is considered a leader in the area of geriatric educational training for Augusta University.
Job Summary
The MDS Coordinator, also known as a MDS Nurse, is responsible for adequately assessing, monitoring and documenting nursing home residents' needs and coordinating personalized resident-driven care based on those assessments. Possess knowledge regarding state, federal, VA, regulations and The Joint Commission guidelines governing long-term care. Ensures that individualized quality resident care is provided for all residents by managing nursing staff assigned to designated nursing units. Conducts the weekly interdisciplinary meetings. Communicates effectively with residents, family members, employees, and other health care providers. Possess strong leadership and organizational skills and the ability to multi-task.
Responsibilities
The duties include, but are not limited to:
RESIDENT ASSESSMENT: Assesses resident's behavioral, psychological, clinical, nutritional, mental, and physical strengths to create individualized care plans that considers the resident holistically. Responsible for developing care plans on all residents based on assessments at the time of initial admission, readmissions, quarterly, when significant changes occur-both improvements and declines, and annually.
COORDINATES THE DEVELOPMENT AND COMPLETTION OF THE RESIDENTS: Coordinates the development and completion of the residents' assessment (MDS) in accordance with current regulations and guideline. Works with the ICT to Develop care plans that have long-term and short-term goals for improvement. Participates in facility surveys made by federal, state, and The Joint Commission.
CONDUCT MEETINGS: Conduct meetings with multidisciplinary/interdisciplinary team, caretakers and families. Works with the interdisciplinary care team to develop an accurate, individualized, comprehensive plan of care. Ensure quarterly and annual resident assessments and care plan reviews are made on a timely basis.
PARTICIPATES IN PLANNING IN-SERVICE TRAINING CLASSES: Participates in planning in-service training classes that include assessment skills or techniques needed to complete assessment functions in LTC Component. Participates in all state, federal VA and Joint Commission surveys.
ADMISSION PREPARATION: Once residents are approved for admission and an admission date scheduled, reconcile medical information with the family/responsible party and if applicable transferring facility to prepare for day of admission. Prepare the admission assessment and interim care plan outline for administration and the IDT.
COMMUNICATE ASSESSMENT TO ALL THE TEAM: Ensure that all members of the interdisciplinary team are aware of the importance of completeness and accuracy in their assessment functions. Monitor nursing documentation and MD orders to ensure accuracy and timely submission for changes that impact care planning.
ABILITY TO COLLABORATE EFFECTIVELY WITH MEMBERS: Ability to collaborate effectively with members of the interdisciplinary care team (ICT) to include the Medical Director, Physician Assistant, nursing, PT, OT, nutrition/dietician, activities, and social work services to ensure care plan guidelines for each resident are followed and compliant with ethical standards and regulatory requirements.
OTHER DUTIES: Perform all other job-related duties as assigned.
Required Qualifications
Graduate from an accredited Nursing college or university; Current, Active Georgia Licensure as a Registered Nurse in good standing; Current American Heart Association Healthcare Provider BLS/BCLS certification; Ability to learn on-the-job training towards MDS certification.
Preferred Qualifications
On-the-job training experience or completed and MDS-training program and are familiar with MDS 3.0 and have an in-depth working knowledge of the Resident Assessment Instrument (RAI) user manual and RUG categories.
Knowledge, Skills, & Abilities
KNOWLEDGE
Knowledge of AU/GWVNH policies and procedures
Proficient in Microsoft Office and other computer software/databases
Considerable knowledge of principles and practices of nursing as it applies to the geriatric population
SKILLS
Strong telephone etiquette skills
Excellent interpersonal, written and verbal communication skills
ABILITIES
Ability to maintain confidentiality
Ability to access and accurately report changes in patient's condition
Certifications/License
Active Georgia Licensure as a Registered Nurse in good standing.
Current American Heart Association Healthcare Provider BLS/BCLS certification

All selected candidates are required to successfully pass a Background Check review prior to starting with Augusta University.
In conjunction with an offer of employment, selected candidate(s) will be required to successfully complete Augusta University's Criminal Background Check process. In addition, candidates selected for any position within Georgia War Veteran's Nursing Home (GWVNH) will be required to successfully complete the Georgia Long-term Care Background Check process. For specific positions, selected candidate(s) may also be required to successfully complete supplementary pre-employment onboarding requirements such as: a physical, drug screening and/or credit check.
If applicable for the specific position based on the duties: the candidate will also need to have a credit check completed for Positions of Trust and or approved departmental Purchase Card usage.
Motor vehicle reports are required for positions that are required to drive an Augusta University vehicle.
For Faculty Hires: Final candidates will be required to provide proof of completed academic degree(s) as well as post-secondary coursework in the form of original transcript(s). Those candidates trained by a foreign institution will also be required to provide an educational/credential evaluation.
