Position Description:

The Division of Infectious Diseases is seeking to hire a Behavioral Health Care Manager. The Behavioral Health Care Manager will provide comprehensive Behavioral Health Services in support of Ryan White Grant eligible individuals and their families. The Behavioral Health Care Manager will help individuals eliminate access issues, which enables them to lead healthier, and more productive lives through the provision of behavioral health screener, group and individual therapy, crisis intervention, and outreach services throughout the 550 Clinic service area.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Conduct initial patient intake assessments and refer patients, clients, or family members to community resources or to outside Mental Health Professionals or other agencies as necessary.

  • Provide clinically appropriate follow-up visit to behavioral health screening, emergency intervention(s), and treatment plan as necessary.

  • Assess participants' mental condition based on review of client information, interviews, observation, tests, and collaboration with other care team members or external partner.

  • Meet with clients regularly to provide individual and/or group psychotherapy sessions, crisis interventions, and adjustments to individualized care plans (ICPs) as necessary.

  • Conduct and document ongoing patient progress.

  • Maintain progress notes and other documentation required for the patient in EPIC Chart (EMR).

  • Participate as an active member of the clinical treatment team, including participating in case team meetings, sharing expertise and clinical impressions with care team members, and attending staff meetings and trainings.

  • Provide feedback on clinical and diagnostic matters to clinical management staff.

  • Develop and implement treatment plans according to patient needs, clinical experience, and knowledge.

  • Serve as a client advocate to coordinate required services or to resolve emergency problems in crisis situations.

  • Gather information about community mental health needs and resources that could be used in conjunction with therapy.

  • Help plan referral activities in surrounding communities to better identify and establish more appropriate mental health resources in program service area in collaboration with Grant Program Leadership.

  • Plan and create programs to prevent substance abuse or improve community health and counseling services in collaboration with Grant Program Leadership.

  • Participate in quality initiatives and utilization review activities as requested.

  • Coordinate services with representatives of Cardinal Success Program.

  • Clinically supervise practicum students placed at KCCP and Ryan White 550 Clinic in coordination with the supervisor at Cardinal Success Program.

  • Meets quarterly with leadership team from Ryan White and Cardinal Success Program to evaluate quality of patient care.

  • Other duties as assigned.

Target Compensation Maximum:

$87,300.00

Target Compensation Minimum:

$58,200.00

Compensation will be commensurate to candidate experience.
