The Trager Institute is seeking a dedicated and compassionateSocial Worker II to join our team and make a meaningful impact on the lives of older adults in communities across Kentucky. The position will be supporting the Area Agencies on Aging (AAA) in Barren River. Responsibilities include bridging the gap between clinical care and community services, ensuring that older adults receive comprehensive and patient-centered care, and utilizing data collection skills toensure seamless collection of outcomes. Responsibilities include ensuring programs align with the latest evidence-based practices and meet the specific needs of individuals with Alzheimer's and related dementias (ADRD), delivering training on dementia care for healthcare and supportive care workers, leveraging resources to enhance skills and understanding of ADRD. This individualwill also be responsible for guiding organizations to become moreage-friendly when it comes to providing care for adults experiencing ADRD.
The position is grant funded and are contingent upon the renewal of those funds.
Key responsibilities include:
Coordinate case management services helping bridge the gap between clinical care and community resources.
Provide support to older adults and their families helping them navigate healthcare systems and access community resources.
Close coordination with the Area Agency on Aging to ensure that patients are receiving appropriate care and services.
Advocate for the needs and rights of older adults within the healthcare system and community.
Supervise and mentor social work students during their field placements, providing guidance, feedback, and professional development opportunities.
Participate in data collection methods and work closely with the data team to ensure outcomes are being met.
Deliver comprehensive training programs on dementia care for healthcare and supportive care workers, enhancing their skills and understanding of ADRD.
Monitor and evaluate the effectiveness of dementia care programs, making adjustments as needed to improve outcomes for individuals with ADRD.
Ensure all programs and services comply with relevant regulations and standards, maintaining high-quality care for individuals with ADRD.
Preferred Qualifications:
Bachelor's in social work.
Ability to remain flexible.
Strong understanding of age-friendly healthcare principles and practices.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Experience with data collection and reporting.
Must be located in or willing to commute to the Area Agencies on Aging (AAA) in Barren River (serving Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson, and Warren counties).
Target Compensation Maximum:
$68,900.00Target Compensation Minimum:
$45,900.00Compensation will be commensurate to candidate experience.
