Job Description

Summary

The Community Health Worker (CHW) will receive training to work with a local food assistance program to provide a behavioral family intervention in a federally-funded behavioral research study. The study is focused on developing and implementing an intervention for families of children (ages 10-15 years) with type 1 diabetes who have food insecurity. The Community Health Worker will deliver the intervention. Training will be provided in intervention delivery.

Job Duties

  • Conducts behavioral intervention services, targeting families of children with food insecurity.
  • Refers and links families to a food assistance program.
  • Documents each patient served, documents progress, and maintains data collection logs and form.
  • Provides culturally and linguistically appropriate services.
  • Maintains a professional disposition while working with a multidisciplinary health care team.
  • Completes accurately, and in a timely manner, all-necessary forms, case notes, and reports, and submits such documentation to his or her supervisor within designated timelines.
  • Participates actively in regular supervisory and team meetings, training sessions, conferences, seminars and independent study. Participates in program and operational planning.
  • Follows-up with clients regarding appointments, care plans, and health goals.

Minimum Qualifications

  • High Schooldiploma or GED.
  • Two years of relevantexperience in health/social services.

Preferred Qualifications

  • Bachelor's degree in Social Work.
  • Bilingual (English/Spanish).

Work Authorization Requirement:
